Usage of ICOI Facilities
Please review the Facilities Usage Policy before filling out a request form. By filling out the request form, you agree that you have read and will abide by the policy.
ICOI Facility Request Form
If you would like to hold a gathering at ICOI, please print and fill out the following forms: Facility Request, Regulations, Facility Setup. (PDF reader required). You can either fax it to us at 949-786-4265 or bring it and and submit it at the front desk.

Want Sheikh Sadullah Khan to attend your event:
In order to invite Sheikh Sadullah Khan to a gathering, kindly download and print the Request Form, fill in the form thoroughly, and fax it to (949) 786-4265. Forms can also be mailed to: "Request Sheikh Form, 2 Truman, Irvine, CA 92620". All Requests should be faxed/mailed at least 21 days prior to the date requested. ICOI will accept or respectfully decline your request wihthin 48 hours from the day we recieved your form. Jazakallahkhair.
